Pre-referral Questionnaire
Thank you for taking the time to complete the screening questionnaire.
Unfortunately, your responses to the screening questions indicate that this referral does not meet the eligibility criteria for Embark.
All Referrals to Embark must include a copy of the Embark Consent Agreement Form signed by the participant. Eligible Embark participants must also meet each of the following criteria:
- Be linked with services in the greater Sydney area
- Have a mental health condition
- Be at risk or currently experiencing homelessness
To be eligible to be supported by NDIS:
- Age : To be between 18 and 65
- Residency : live in Australia and an Australian citizen or have a Permanent or Special Category Visa.
Your nearest NDIA office can help put you in contact with an ECEI or LAC partner or help with applying for the NDIS.
