What is Support Coordination?

Support coordination is a capacity building support designed to assist NDIS participants to get the most out of their NDIS plans. We work closely with you to help you understand your NDIS plan identify suitable service providers, and facilitate connections to ensure that your needs and goals are met.

How does ICLA Support Coordination work?

At ICLA, we work closely with you to navigate the complexities of NDIS, empowering you to make informed decisions about your supports and services. Our support coordinators will work with you to maximise your NDIS funding and ensure that tailored, individualised supports are implemented for your specific needs and goals. We’re here to help you access supports that work for you, whether it’s engaging in community activities, perusing employment opportunities, cultivating meaningful relationships or managing daily activities. Our goal is to help you lead a more independent life.

What makes our Support Coordinators different?

If you’re the parent, sibling, friend, or carer of a loved one with a NDIS plan, you’ll understand just how important it is to work with people who understand your needs. That’s why we ensure our Support Coordinators are equipped with the right skills to help your loved one thrive.

All our Support Coordinators are:

  • Combined experience of 10 + years in Support Coordination.
  • Highly empathetic, compassionate, dedicated, and person-centred.
  • Concerned with improving the quality of clients’ lives.
  • Detail-oriented, with a track record of delivering high quality specialised services.
  • Have experience with working with people with complex mental health and psychosocial disabilities.
  • Advocates for client choice, independence, recovery, and well-being.
  • Equipped with a deep understanding of the mental health and NDIS systems, including accessing appropriate assessments, securing funding, and identifying suitable services that support adults with complex needs.

What levels of Support Coordination are offered in a NDIS plan?

Your plan may include three different levels of support coordination:

Support Coordination – Coordination of Supports: This level of support is designed to equip you with the necessary skills to comprehend and use your plan efficiently. Here you’ll receive a balanced mix of supports that enable you to maintain relationships, manage service delivery tasks, foster greater independence, and actively participate in your community.

Support Connection: This service focuses on strengthening your ability to connect with informal, community, and funded supports. By utilising these connections, you can improve your plan and work towards achieving your goals more effectively. Specialist Support Coordination: Reserved for individuals facing more complex situations, this higher level of support coordination provides specialised assistance. Specialists who are well-versed in NDIS disability support services will help you navigate challenges within your support environment. They ensure the consistent delivery of services tailored to your specific needs and circumstances, helping you maximise your overall well-being and independence.

Am I eligible for Support Coordination?

Our Support Coordination is available for people who:

  • Live in Sydney
  • Have Support Coordination funding as a part of their NDIS plan
  • Have a diagnosed psychosocial disability and/or mental health condition.

How to get started?
