Aged Care Services, provided under the CHSP

The services we provide to Aged Care participants under the Commonwealth Home Support Programme (CHSP) provide entry-level support to help older people continue to live safely and independently at home and in their communities. The CHSP supports activities that enable independence within the home, and that consider each client’s individual goals and choices.

What is the ICLA Commonwealth Home Support Programme ?

The ICLA Commonwealth Home Support programme (CHSP) is a functional capacity building initiative directed towards older individuals who need support to regain and maintain independence at home. If you are an older person who is in need of individualised support this programme can provide a range of low-level support services tailored to your needs, allowing you to achieve individual goals towards independence. It provides compassionate and professional assistance that empowers you to live comfortably and confidently in your own home.

What support does the Commonwealth Home Support Programme offer?

The Commonwealth housing support programme provides a range of entry-level support to promote wellness and reablement on a short-term basis, with the specific aim of helping individuals to return to doing home maintenance and domestic tasks independently. Services tend to be delivered within a 12-week period with the aim of wrapping up services when the client has met their goals or specific outcome. The programme is primarily funded by the Australian government, which allows ICLA to deliver high quality subsidized services to older people who live in Australia. In addition to government funding Individuals receiving CHSP services are asked to contribute towards the cost of the services they receive. This contribution is referred to as a ‘client contribution’ and varies depending on the individual’s financial situation.

As you get older life throws new challenges at you, and sometimes you need some support to embrace the journey of aging with confidence. The CHSP can help you to maintain your independence and continue living life on your own terms. CHSP is different to other programs as it delivers a broad spectrum of subsidized services, that can be tailored to meet the immediate and short-term needs of an individual. The flexible and straight-forward service delivery allows individuals to access support easily through the ‘My Aged Care’ portal and allows individuals to continue to receive support when circumstances change. The programs emphasis on reclaiming independence so as to support older people to stay connected and engaged in their local communities and in the activities that they enjoy is key to promoting individual wellness and reablement.

How It Works

After registering with ‘My Aged Care’ clients can access the online portal to request community and home support.
Clients connect with reliable and passionate domestic assistants who deliver client-centered support.
Clients actively work on achieving their individualised goals and overall wellbeing through functional capacity building.
Maintaining regular communication with service provider and domestic assistants to discuss any change of circumstances and progress towards goals.

Am I Eligible for the Commonwealth Home Support Programme?

To receive support through the Commonwealth Housing Support programme, you must gain access to the ‘My Aged Care’ portal. To gain access to the portal individuals will need to call the ‘My Aged Care’ contact Centre on 1800 200 422 (free call) between 8:00am- 8:00pm on weekdays and between 10:00am – 2:00pm on Saturday’s. Alternatively, individuals can visit their local Service Australia service Centre and book an appointment with an Aged Care Specialist Officer (ACSO).

‘My Aged Care’ is the entry point for Australian Government-funded aged care services.

The following groups are eligible for CHSP:

  • Frail older people aged 65 years and over (Or 50 years and over for Aboriginal and or Torres Strait islander people) who:
        – Need assistance with living independently at home and participating in the community or
        – Need planned respite services so their carers can have a break from their duties.
  • Frail people or prematurely aged people 50 years and over (or 45 years and over for Aboriginal and Torress strait islander people) on a low income who are:
        – Homeless or at risk of homelessness as a result of experiencing housing stress or not having secure accommodation or
        – Living with hoarding behavior or in a squalid environment, and at risk of homelessness or unable to receive the aged care services they need.
